Veterans and war widows can receive funding through the Department of Veterans Affairs to help with health and nursing needs at home.

How it works

Step 1

Understanding eligibility

Veterans and war widows with a Gold Card and the need for nursing at home care eligible for DVA Community Nursing support. White Card holders may also be eligible if your condition is war related. 

You’ll simply need a referral.

Step 2

Get a referral

To access DVA Community Nursing, you need a referral from one of the following: 

  • GP – D904 Referral Form required
  • Hospital discharge planner
  • Treating doctor in hospital
  • Veterans Home Care Assessment Agency
  • Nurse practitioner specialising in Community Nursing

Step 3

Get services started 

Once you have your referral, simply call our office on 1300 770 118 to talk to our Registered Nurses to get your services started.
