The Veterans Home Care (VHC) assists veterans and war widows/widowers with low level needs who wish to continue living at home for longer and avoid premature admission to residential care. The program is provided through the Department of Veterans’ Affairs (DVA)
Veterans and war widows are eligible for a VHC assessment for all VHC services. In order to receive services a veteran must be assessed by the Veterans’ Home Care Assessment agency for their area.

The Assessor will talk to you about your situation at home and identify services you may need.
Following a request from the VHC assessment agency Harbour At Home will arrange a time with you to commence the delivery of the approved services.
If you’re eligible, the services you need will be detailed in a VHC care plan, which will be sent to you and to Harbour At Home as your provider. This plan will include:
details of what is approved
the period of approval
the co-payment amount

A Member of the Aged Care Assessment Team (ACAT) will visit you in the home to carry out an assessment to determine what level of care is most suitable to you and your situation.
There are 4 levels of care through Home Care packages ranging from Level 1 (low level) to Level 4 (high level).

Once you have received your Home Care Package our Service Planners will listen to what’s important to you and develop a Support Plan in consultation with you, your family or carer. You determine your level of involvement in how your home care package is used, so that you can obtain the support and lifestyle you want.
We ensure that we work closely with the person that has been assigned the Package, their family or carer. We discuss their health & wellness goals whilst ensuring the support received suits the person’s lifestyle choices and daily routines. Our wellness and reablement approach to service delivery aims to promote independence and autonomy.
We are competitive with our Package Management and Care Management charges ensuring your package of care and support is not taken up with excessive costs in order to maximise the available funds to support your needs.

The cost of our service will depend on a client’s program and funding package. For some programs there may be a small gap payment, while for other programs there may be none. This is why we work with you to find the right Home Care worker, one which you can build rapport and how is best suited to undertake your home care requirements. Our Care team also work with you to build a schedule by giving your choice and options.
