A Home Care Package is a government-funded program that provides access to support services to help you stay in your own home and remain connected with your local community. They give you a choice and flexibility in the way your support services are delivered.
There are 4 levels of care through Home Care packages ranging from Level 1 (low level) to Level 4 (high level).
My Aged Care will send you a letter with the outcome of your assessment, this will specify if you are eligible for a Home Care Package and what level of care you will receive. You will then be placed on a national waitlist and notified when your package becomes available.
Three months prior to your package becoming available you will receive a ‘Readiness Letter’ from My Aged Care. This is an important letter, there is paperwork you need to complete. Contact us on 1300 770 118 when you receive your letter and we will guide you through the next steps.
Once you have received your Home Care Package our Care Managers will listen to what’s important to you and develop a Support Plan in consultation with you, your family or carer. You determine your level of involvement in how your Home Care Package is used, so that you can obtain the support and lifestyle you would like.
We ensure that we work closely with the person that has been assigned the Package, their family or carer. We discuss their health & wellness goals whilst ensuring the support received suits the person’s lifestyle choices and daily routines. Our wellness and reablement approach to service delivery aims to promote independence and autonomy.
We are competitive with our Package Management and Care Management charges ensuring your package of care and support is not taken up with excessive costs in order to maximise the available funds to support your needs.

When your Home Care Packaged becomes available you will receive another letter from My Aged Care. You will have 56 days from the date your package is assigned to accept the funding and commence your care plan.

If you fail to accept your funding before the deadline, your package will be withdrawn. Please contact us if this happens and you wish to apply again.

Bill has Alzheimer’s Disease and is slowly losing the ability to do the things he has loved to do in the past. Things like camping, travelling, reading, woodwork, gardening to name a few. This is very frustrating and depressing for him.
In his prime Bill was a very good and successful swimmer both in the pool and the surf club where he was chosen in the state team.
Recently, we approached Harbour At Home which enabled Bill to access swimming sessions at Coffs Olympic Pool through his Home Care Package.
It is marvellous. Bill now benefits from the regular exercise the swimming provides, but more importantly, he benefits from the sense of pride and achievement that he can still swim!

Our Care team works with you to build a schedule with choice and options. We do this by working with you to build a roster, that suit you and your family best, in line with home care worker availability.
We at HSS, know how important consistency and regularity is to how you receive your home care services. This is why we work with you to find the right Home Care worker, one which you can build rapport and how is best suited to undertake your home care requirements. Our Care team also work with you to build a schedule by giving your choice and options. Once a roster has been worked out, we will provide this to you on a regular basis, so you know who is coming and when.
