Harbour At Home has been delivering in-home nursing services to eligible veterans since 1991. Since then we have had the privilege of providing DVA community nursing services to hundreds of veterans, war widows and war widowers on the Mid North Coast assisting them to maintain their independence and providing them with the choice of being cared for at home.
The duration and frequency of visits required to a veteran will be determined upon the assessment of the client’s clinical needs. The services provided will depend on the client’s assessed clinical care needs and include:
Veterans and war widows or widowers who have assessed need for care at home and a: Gold Card are eligible for DVA Community Nursing. White Card holders may also be eligible for this service where the condition requiring nursing care is accepted by DVA as war related.
To access Department of Veteran Affairs Community Nursing, you will need to get a referral to Community Nursing services from a:
General practitioner (GP) D904 referral form required.
Hospital discharge planner; or
Treating doctor in hospital

A Member of the Aged Care Assessment Team (ACAT) will visit you in the home to carry out an assessment to determine what level of care is most suitable to you and your situation.
There are 4 levels of care through Home Care packages ranging from Level 1 (low level) to Level 4 (high level).

Once you have received your Home Care Package our Service Planners will listen to what’s important to you and develop a Support Plan in consultation with you, your family or carer. You determine your level of involvement in how your home care package is used, so that you can obtain the support and lifestyle you want.
We ensure that we work closely with the person that has been assigned the Package, their family or carer. We discuss their health & wellness goals whilst ensuring the support received suits the person’s lifestyle choices and daily routines. Our wellness and reablement approach to service delivery aims to promote independence and autonomy.
We are competitive with our Package Management and Care Management charges ensuring your package of care and support is not taken up with excessive costs in order to maximise the available funds to support your needs.

After Dave had a stroke in 2013 and spending 15 months in hospital and several hospitalisations over the last several months he is finally home. The cost of our service will depend on a client’s program and funding package. For some programs there may be a small gap payment, while for other programs there may be none. We can guarantee that unlike other Home Care providers we have no hidden extra fees. Our Care team works with you to build a schedule with choice and options. We do this by working with you to build a roster, that suit you and your family best, in line with home care worker availability.
We at HSS, know how important consistency and regularity is to how you receive your home care services. This is why we work with you to find the right Home Care worker, one which you can build rapport and how is best suited to undertake your home care requirements. Our Care team also work with you to build a schedule by giving your choice and options. We do this by working with you to build a roster, that suit you and your family best, in line with home care worker availability.
Once a roster has been worked out, we will provide this to you on a regular basis, so you know who is coming and when.
Our Home Care team will work with you to build a team of local home care workers, who are great at what they do, as well as knowing your needs and requirements inside and out. Once we have worked out with you, who the local members of your Home Care team are, we will send you a schedule so you know who is coming and when.
