What is it?

In Home Flexible Respite Care is when someone else takes care of the person you care for so you can take a break.

At Harbour At Home we offer services funded by the Australian Government’s Commonwealth Home Support Program (CHSP).

Caring for someone can be tiring and at times stressful, caring for yourself as well as your friend or family member is important.

Here at Harbour At Home, we know how important carers are to helping people with Dementia or someone who is frail aged. We recognise that all people are unique and we will tailor your respite support according to your needs. We also consider the needs of the person being cared for so that the respite is worthwhile to both parties and has positive benefits to both the carer, and the person being cared for.

Eligibility for In Home Flexible Respite

If you are a carer of someone over the age of 65 you DO NOT have to be a registered carer with Centrelink to qualify for in home flexible respite support? There are many people who care for someone who may not even recognise that they are a carer or that they are entitled to some support to assist them in their caring role.

STEP 01

Carer Gateway

Carer Gateway is a free, Australian Government service, which provides information about the support available to carers.

Click here to visit the Carer Gateway website or give them a call on 1800 422 737 (8am – 6pm, Mon-Fri).

STEP 02

My Aged Care

If the person you care for is over 65 years old (or over 50 if Aboriginal or Torres Strait Islander) you might be able to access In Home Flexible Respite Care by calling My Aged Care on 1800 200 422 and ask for Harbour At Home as your preferred provider.

STEP 01

Contact My Aged Care

To request an assessment you can call 1800 200 422. Or you can apply online via the My Aged Care website. Click here to go to the online application form. You will need your medicare card to complete this step. Contact us after you have spoken with My Aged Care. We can help you to prepare for your assessment and organise any required support in the mean-time with private funding. You can nominate a service provider such as NSW-Harbour At Home when requesting your assessment. This means they will notify us when you’re package becomes available, so we can contact you and start organising your care plan.

STEP 02

Face-to-face Assessment

A Member of the Aged Care Assessment Team (ACAT) will visit you in the home to carry out an assessment to determine what level of care is most suitable to you and your situation.

There are 4 levels of care through Home Care packages ranging from Level 1 (low level) to Level 4 (high level).

STEP 03

Assessment Outcome

My Aged Care will send you a letter with the outcome of your assessment, this will specify if you are eligible for a Home Care Package and what level of care you will receive. You will then be placed on a national waitlist and notified when your package becomes available. Three months prior to your package becoming available you will receive a ‘Readiness Letter’ from My Aged Care. This is an important letter, there is paperwork you need to complete. Contact us on 1300 770 118 when you receive your letter and we will guide you through the next steps.

STEP 04

Accept Package & Choose Provider

When your Home Care Packaged becomes available you will receive another letter from My Aged Care. You will have 56 days from the date your package is assigned to accept the funding and commence your care plan. Please contact us on 1300 770 118 when you receive this letter. If you fail to accept your funding before the deadline, your package will be withdrawn. Please contact us if this happens and you wish to apply again.

STEP 05

Create Your Care Plan

Once you have received your Home Care Package our Service Planners will listen to what’s important to you and develop a Support Plan in consultation with you, your family or carer. You determine your level of involvement in how your home care package is used, so that you can obtain the support and lifestyle you want.

We ensure that we work closely with the person that has been assigned the Package, their family or carer. We discuss their health & wellness goals whilst ensuring the support received suits the person’s lifestyle choices and daily routines. Our wellness and reablement approach to service delivery aims to promote independence and autonomy.

We are competitive with our Package Management and Care Management charges ensuring your package of care and support is not taken up with excessive costs in order to maximise the available funds to support your needs.

Answers to common queries

The cost of our service will depend on a client’s program and funding package. For some programs there may be a small gap payment, while for other programs there may be none. We can guarantee that unlike other Home Care providers we have no hidden extra fees. To learn more about how much your service may cost, please get in contact with our local team on 1300 770 118 and they will be happy to discuss.

Our Care team works with you to build a schedule with choice and options. We do this by working with you to build a roster, that suit you and your family best, in line with home care worker availability.

We at HSS, know how important consistency and regularity is to how you receive your home care services. This is why we work with you to find the right Home Care worker, one which you can build rapport and how is best suited to undertake your home care requirements. Our Care team also work with you to build a schedule by giving your choice and options. We do this by working with you to build a roster, that suit you and your family best, in line with home care worker availability.

Once a roster has been worked out, we will provide this to you on a regular basis, so you know who is coming and when.

Our Home Care team will work with you to build a team of local home care workers, who are great at what they do, as well as knowing your needs and requirements inside and out. Once we have worked out with you, who the local members of your Home Care team are, we will send you a schedule so you know who is coming and when.

Yes, this is possible. If you are currently unhappy with your Home Care provider and would like to switch to New South Wales HSS, we can guide you through this process. To learn more call our local team on 1300 770 118.

Yes! Here at New South Wales HSS unlike other providers, we don’t believe in national call centres. When you call us, you are put through to our local team of Care Managers who will be happy to chat.
